Faculty and Professional/Classified Staff with benefits and one full year of service are eligible to apply.
1. Obtain application from from an above link or contact any Upward Mobility Representative.
2. Complete application fully. Incomplete applications will be returned
without consideration. Obtain
the signature of immediate supervisor approving work adjustments in instances
of course conflict.
3. Submit application (along with a degree outline approved by degree advisor
as applicable and
copy of bill or information showing tuition costs) to your Upward Mobility
Representative as indicated
below. If you are not seeking a degree or professional certification,
please attach a memo of
explanation for seeking upward mobility funds.
4. Retain a copy of your application and related materials.
GUIDELINES FOR RECEIPT OF AWARDED FUNDS:
1. Complete course with acceptable grade (c or better for undergraduate
course; b or better for graduate
level course; passing for certificate course).
2. Submit a “Memorandum” with your name, SSN and a college or district
department of assignment.
Be sure to sign the memo and attach a copy of your grade report and a copy
of the institution’s bill
for the course work.
3.
Beginning
with Spring 2002 all reimbursements will be made via Accounts Payable (PVQs).
