The
online
version of the appraisal
form should be opened and saved to your hard drive. You
can then open the form as a Word document. This will allow
you to complete the form at your convenience. More instructions
are included in the appraisal form and in the online training.
You can not
type into the printable
version of the
appraisal form. The form is for the supervisor who feels
more comfortable hand writing the information into the form.
It can also be used for the employee self evaluation.
The
performance
measurement definitions & examples
are available to assist you in completing the performance measurement
section of the appraisal form. If you will be using the
printable version you should print this document at the same
time you print the appraisal form. If you will be using
the online version you can copy and paste the examples from
this document into the appraisal form.
The
job
descriptions link
will take you to the Human Resources web page where you can
locate the job description(s) for the employee(s) that you are
evaluating. If you are using the printable version of
the appraisal form you will need to print out the employee's
job description. This will allow you to copy the pertinent
information into the appraisal form.
The
performance
event file can be
used to document positive and negative events throughout the
year.
The

worksheet can
be used as a short employee self evaluation.
