HAZLEWOOD APPLICATION & ELIGIBILITY REQUIREMENTS
1. Submit the Free Application for Federal Student Aid (FAFSA) form to federal processor. The processor will mail you a Student Aid Report (SAR) within four (4) weeks, submit the SAR and any other forms as required to the Student Financial Services (SFS) Office.
2. Obtain and submit an authorization letter from the V.A. Office located at 3601 Bluemel (I-10 and Wurzbach), indicating your V.A. educational benefits have been depleted and you are no longer eligible for future benefits.
3. Submit a copy of your DD214, under section discharge, it must state Honorable,General Under Honorable, or Honorable Conditions. These are the only acceptable status for Hazlewood benefits.
4. Must have entered service from Texas and DD214 indicates permanent resident of Texas (PL.54.203A) at time of entering and must have serviced 180 days (6 months) active duty excluding time in basic training.
5. If application IS complete at time of registration, the SFS Office issues a payment voucher to the student. The student submits the voucher to the Bursar for payment of the registration bill.
6. If the application IS NOT complete at time of registration, the student must pay the registration bill. Once the application is complete and eligibility verified, the SFS Office issues a payment voucher to the student. The student submits the voucher and copy of paid bill to the Bursar for reimbursement.
7. Tuition and fees for up to 150 semester hours of credit (or it’s equivalent) courses. The 150 hour limit applies to all course work taken beginning with the Fall, 1995 semester. No books and supplies can be paid through Hazlewood.
8. The SFS Office does not need a copy of the tuition bill after it has been marked paid.
9. List below all colleges, universities, and vocational/technical schools attended (since Fall, 1995) where you received Hazlewood benefits. (Use back of form if additional space is needed.)
