Accommodations
The Disability Support Services staff will help you fill out these forms
to request accommodations:
- Request for Services
- Release of Information
- Reader/Scribe & Taped Reading Request
- Adaptive Equipment
Request
- Extended Test Time Arrangements
- Interpreter Request
Request for Services
All students must fill out a "Request for Services" form
and make an appointment with the Coordinator
EACH SEMESTER. This form will be reviewed during your intake interview.
Release of Information
Any student who cannot provide Disability Support Services with up to date disability documentation, must sign a "Release of Information" form,
so Disability Support Services can legally request disability information
from other agencies. This form can be obtained from Disability Support Services.
The types of information Disability Support Services might request are:
- Documentation of Disability
- Medical Documentation
- Psychological Evaluation
- Academic and/or Intellectual Assessments
- Current or Previous
Work Related or Educational Accommodations
Reader/Scribe & Taped
Reading Request
If eligible students needing a Reader/Scribe
or would like to request class materials read onto tape, simply
need to fill out
a request form.
Adaptive Equipment Request
Eligible students who need Adaptive Equipment simply need to come
into Disability Support Services and fill out a request form.
Extended Test Time Arrangements
Disability Support Services does have does have reserved testing sites
and times for eligible students needing Extended Test Time Arrangements.
Instructors may make their own arrangements directly with the student
for the Extended Test Time accommodation. If the student will be
using Disability Support Services' services for Extended Test Time Arrangements,
he/she needs to come to Disability Support Services to fill out the appropriate
form. The student will also bring an Instructor's Test Directions
form to the instructor for completion.
Interpreter Request
Any student who qualifies for Interpreting Services, must
fill out an Interpreter Request form:
- at least 2 weeks before the
semester starts for in class interpreting.
- any time they need
an Interpreter for an out of class meeting or activity.
- out-of-class
requests must be made at least four days
in advance.
