These requirements are intended to assist chairpersons and 
administrators in the evaluation of future Internet course 
development and the selection of Internet course instructors.

Have your instructor stamp the appropriate box, initial, and date it after completing each of the items above.
Courses may  possibly be waived if competency is demonstrated.

NOTICE: When all requirements have been completed, keep a copy of this sheet and turn in
the original to Charlotte Wolf at San Antonio College, MLC 716, in order to become certified.

 
NOTICE: Certification Fees
  This training and certification was planned for employees of the Alamo Community College District (ACCD).
1.
No fee is charged to employees of the (ACCD) for this "Internet Administrator's Certificate."
  At the time you request certification, you will be required to provide verification of your employment in the ACCD and completion of all requirements.

If you are not an ACCD employee, you may receive this "Internet Administrator's Certificate" by completing all requirements and paying a fee of $120 to the Department of Instructional Technologies.
