Due to clinical facility contractual agreements that require student health insurance coverage, we are instituting the following policy:

All nursing students, before starting clinicals in the Fall, must show proof of Health Insurance Coverage to their clinical instructor.

The instructor makes a copy of the proof and submits this to be placed in the student's file.

THEREAFTER, STUDENTS MUST PROVIDE PROOF OF HEALTH INSURANCE COVERAGE

• at the time of admission, and
• upon renewal of the Hospital Policies Orientation Card.

This record must be kept in the student's file.
