COURSE ALIGNMENT FORM

1. Include your Department/Program Name2. Include Course Rubric and Course Number

3. Include Course Title

4. Include course description

  • If you are adding a new course use green font
  • If you are deleting a course use red font
  • If you are modifying course title, prerequisite, description, or semester credit hours use blue font to identify Current and Proposed changes

5. Include CIP CODE for WECM changes or Course Approval CIP for ACGM changes

6. Include Basic Skills Requirements

7. Include Prerequisites, Co-requisites, Cross-listings

8. Include Semester Credit Hours

9. If course is listed in current catalog include page number

10. State yes/no if there is a change in the course

10. Email electronic copy to bguerra30@mail.accd.edu
