APPLICATION PROCESS, GUIDELINES & FORMS
Students desiring to major in an Allied Health Program must first meet
admission requirements for acceptance to St. Philip's College at the college
level.
A separate application process is required for admission into any Allied
Health program of study. This application process includes documentation on
student identification, eligibility and prerequisite requirements, and statement
of reasons for interest in an Allied Health program.
Additional criteria and program prerequisites have also been established by
each Allied Health program. To be eligible for admission to an Allied Health
program of study, prerequisites must be completed prior to admission to the

The Allied Health Application Process includes documentation of the following
items:
- Application Form with
identification information;
- Assessment scores verifying acceptance to St. Philip's College;
- Unofficial college transcripts verifying completion of prerequisites;
and
- One-page typed statement of reasons for interest in program

After the completed application packet with all supporting documents has been
received and reviewed by the Program, additional application requirements may be
requested to meet program admission requirements. These additional requirements
may include a personal interview, pretesting, criminal background checks, drug
screening, or other requirement. Students will be contacted by each program
regarding receipt of application and additional requirements.
