Vocational Nursing Program Application Process
Placement Tests
Potential applicants must schedule and take a placement test and meet the
minimum scores
required before an
application is issued. After testing is complete, scores must be interpreted by
the Counseling Department. NOTE: For testing dates and times, please
contact Advising and Assessment at (210)-531-3263. Applicants who do not
meet admission requirements based on low test scores should see an academic
advisor and discuss taking developmental courses such as English, Reading and/or
Math prior to applying.
The following are accepted placement tests that applicants may take:
• ACT - American College Test
• SAT - Student Aptitude Test
• ACCUPLACER
• ASSET
• TASP/THEA
The student application admission packet must be completed and returned
with the following items:
Transcript Request Form: Please submit two official transcripts, including
high school and college (if applicable); if submitting a college transcript it
must indicate the high school attended and the date of graduation. One
transcript is submitted to the Admissions Office, records and registration section and
one copy should accompany your application. You MUST be a high school graduate or have
your GED Certificate with GED scores.
Request for Reference Forms: Three (3) letters of reference forms must
be completed and submitted (must be current or past employers, supervisors,
coworkers and/or teachers; no family members will be accepted).
Physical Examination Form: This form must be completed and signed by a
physician, physician assistant or nurse practitioner.
Immunization Record: must include documentation of all required
immunizations:
Tetanus
Hepatitis B Series - completed series
Hepatitis A Series (effective January 2008)
MMR (if born on or after January 1, 1957, you must
have a second measles vaccine) (2)
Varicella (chickenpox)
Tuberculosis Skin Test (PPD) with negative results or
chest x-ray report confirming no active case of tuberculosis
Serum titers confirming immunity are accepted for Hepatitis
A, Hepatitis B, Varicella and MMR
CPR Requirement: Proof of current CPR Certification (infant,
child, & adult) from the American Heart Association or Red Cross for Health Care
Providers (no internet courses will be accepted).
Health Insurance: The student must be covered by health insurance for
treating any injuries or illnesses in which the student may encounter while
attending clinical rotations at an assigned health care facility. The
health insurance must be valid for the duration of the Vocational Nursing
Program.
Criminal Background Check and Drug Screen: All applicants must
pass a criminal background check and return with a negative drug screen before
final acceptance into the program. The criminal background check and drug
screening is performed by Alamo Community College District contracted vendors.
Upon successfully completing the Vocational Nurse Program, The
Texas
Board of Nursing (BON) will determine eligibility for candidates to sit
for the National Council Licensure Examination for Practical/Vocational Nurses
(NCLEX-PN).
The BON will perform an additional criminal background check upon completion of
the program and you will be required to answer screening questions to assist
in determining your eligibility for licensure.
The application packet with all required documentation must be completed
before consideration will be given for acceptance into the St. Philip's College
Vocational Nursing Program
Completed application packets must be hand delivered to the St.
Philip's College Nursing Education Department - DO NOT MAIL applications.
You must show proof of identity when turning in your application packet and at that time your
application will be reviewed for accuracy and completeness.
Any applicant requesting consideration into the
Seguin
extension campus must submit their application directly to that program.
The date of submission of the completed application packet will
determine the applicant's place of consideration for acceptance.
