All necessary forms must be printed out and completed prior to submitting for college consideration.
Dual Credit Consent Form - Required for all students entering the Dual Credit program for the first time.
Dual Credit Currently Enrolled Form - Required for students who are continuing the Dual Credit program after their first semester.
Military Verification Form - Required only for students whose parent(s) or guardian cannot claim Texas resident status and are stationed in Texas for military purposes.
